In terms of Baseline bloods, I agree with Empire. You need to know where you stand before a cycle. This would be my suggestion if budget is an issue.
HB, RCC and HCT (can do full blood count if anything is out of range) Why TF would you need a full blood count if this is all you need? Also I would add Haemoglobin (not sure if you call this HCT but here we call it the full name)
Urea and Creatinine (can do a whole UE if you want)

Prolactin Why need this when you're testing Estradiol?
TSH (With reflex T4 thrown in if out of range) Why not rather T4 instead of TSH? Test Tsh after the initial abnormal T4?
HB1AC (as mentioned - not 100% necessary but needed of you have risk factors for T2DM)

Lipogram (Pricey but good to have)
LFT (screening for NASH these days but pricey and not always necessary) If you're Mr Moneybags yeah but why??
