I'd really like some advice for my first cycle (31 years old)
Ive trained on and off from age 18
Height - 171
Weight - 72KG
BF - 15-18%
Calories - 3000
I have access to do my own bloodwork...
Here are some of the important numbers with normal ranges in brackets before starting anything
Test total - 11.8 nmol/L (8-12) "12 or more does not require therapy"
SHBG - 37nmol/L (11.1 -78.1)
Free Test - 220pmol/L (180-739)
FSH - 0.87 U/L (1.0 - 12) LOW
E2 - 48pmol/L (40-161)
Progesterone - 0.3 (0.4-3.1) LOW
Liver, Kidney, Thyroid and Other bloodwork normal
My goal is to add some lean muscle and to gain strength/burn some fat
Proposed Beginner Cycle:
Test Cyp 250mg/Week - 10 Weeks
Anavar 20mg/Day - 8 Weeks
PCT
HCG 5000 (500iu EOD) to start the day of last injection (10 doses)
Nolvadex 20mg Daily for 4 Weeks
Struggling to find PGW online (if anyone can DM a source)..
or I will go with V.Med/EU Pharma/Keifei/UPA
I would like to have kids within the next few years so would like to bounce back - worried about FSH and Progesterone levels
TIA

Here are some of the rest of the results:
LH - 1.0 (0.6 - 12.7 U/L) Normal but on the lower end
Prolactin - 20.6 ug/L (4.3 - 30ug/L)
S-IGF1 - 244.0 ng/ml (41 - 246)
S-IGFBP3 - 5.8 (3.5 - 7)
Hydroxyprogesterone 2.2 (<6.6)
The bloodwork was done about an hour after going to the gym at about 10h00

LH of 1 is not normal. Did you check DHEA levels and maybe IGF so we can see if the pituitary is doing anything right.
If I was you I would firs attempt to get LH and FSH up before you go and turn off signaling all together, but you are pretty much there.
What is interesting is in some cases when the testes is the issue you may have extremely high LH but no testosterone, in your case this seems like an issue with pituitary.
This may need to be looked at by an endo, however you do have some things you can try, but I wouldn't use underground gear to attempt it.
Clomid / AI / HCG (Ovitrelle) and bloodwork every 2 months to make sure you not taking your E2, where it is now is perfect for this exercise. The dosing of those compounds will need to be worked out nicely, but in your case it would make sense to work on the higher end of Ovitrelle administration.

Didn't read that, your IGF is pumping, good show, your DHEA could be higher, that could help things on its own to a degree. Did you test Vitamin D? Strange situation you in, I would honestly try and see if you can get things up naturally. It won't take long to see a difference in your bloods. But again I would probably be doing this through an endo.
Side note: what other medication do you use, have you been extremely overweight in the last few years, do you drink, do you smoke?

I tested smoking as an experimental teenager but never continued.. not my thing
If I drink 1 or 2 drinks every 3-4 months its a lot..
Limit my caffeine intake because I'm a tiny bit sensitive to it.. use it when I need it
Sex drive completely normal. Gf approves lol
I do not take any chronic medication
I have had covid 2020 and had covid vaccine 2021 (March)
I am a carrier of Thalassemia so my rbcs are up a wee bit and a tiny bit microcytic
I only supplement with whey, casein, creatine, beta alanine, citrulline.. the odd Pre-workout and zma
I have not been overweight.. Im actually more underweight or "skinny fat".
Have booked to see my GP during the week, will be referred to Endo I'm sure
Could this be a mistake from Lancet?
Maybe I should retest?
Hoping this isn't a pituitary adenoma...

Skinny-fat you say? All your life or has it gotten worse over time (reduction in lean mass etc)?
Pituitary adenoma or high iron levels in the system could be an issue, a good endo will test these things extensively, before altering hormones.
I suspect should you need pharma intervention, clomid could be potentially useful since you seem to be secondary hypogonadal.

Don't panic about this, but just get some understanding, mayne you have an extra X chromosome, who knows, it's all guessing without being able to nut this out.
I also have some strange issues, not the same as yours as I have ramping LH and FSH, extremely high sperm count but no testosterone.

Due to the low test there is constant signaling for more FSH and LH production..
I'm quite hairy and not underdeveloped so hopefully not XXY.. very decent sex drive.. no loss of facial or body hair.. male pattern balding (unfortunately) and yes there was an oopsy before so I know kids can be produced lol..
But you right before I mess with PEDs which will shut me down

It will also throw off other readings, the endo won't have a problem putting you on prescription test of that's what you guys come up with, but just try get an understanding if there is anything that might need further diagnosis.
