Just was wondering why do some people carry a high body fat percent say anything from 21%-30% but they dont eat as much as one would expect? Also how would one set up a diet for someone whom doesn't eat much and carries a high body fat percent?

Biggest issue these days to me is lack of movement. Majority of people do less than 4000 steps a day. If they pushed that to 8000 a day things would be way different.

Also food choices and lack of protein

Being overweight doesnt necessarily mean they eating a ton of food, they could just not be doing anything physical and not burning calories. They probably have slowed their metabolism down as well which makes it even worse.
If they are not eating much then maybe just validate exactly what they are eating, when they are eating and total calories. If its not totally awful then leave it and just get them on a treadmill (at an incline if possible) and walk for 30 - 60 minutes a day.

It is not elegant or exciting and is uninspiring but its probably where most overweight people should start. These overcomplicated programes can come later, but get them moving, minimize injury risk as much as possible and start burning calories.

There are probably better ways to do this, but this is simple, it has worked for a fair whack of people I know and as long as they are consistent for a few weeks they will start seeing improvements
