Hi guys !!

Hope all is well. I am new here and it's a pleasure to join this community.

I am not experienced in any hormones/peptides what so ever so go easy on me :) I only workout to stay healthy and aesthetics isn't my main goal but just a side effect of my training B)

The main reason I want to start this topic is to ask you guys for some general knowledge and pointers to my current situation, it's always best to have more opinions then just the opinion of your general doctor/s because it feels like the doctors nowadays arn't that knowledgeable when it comes to TRT/hcg etc, I'm not saying my doctor is bad, he is quite cool but i just want more opinions and general pointers to look out for. So let me just get to the point.

For the past 2 years I got extremely anxious and deppresed for some reason, low energy, cognition got worse, sex drive is up and down, my gyno got worse [had gyno since teens] maybe its just more noticeable because i lost weight over the years, my muscle mass went up but extremely slowly over the 2-3 years of training and diet is in check, my eye sight got worse, but what got to me is the low energy, fatigue, sex drive, mental problems and not having a decent pump at the gym, I went to a couple of doctors during this time and they took multiple blood tests.

My last blood test came back
P-glucose fasting - 4.4 mmol/l[3.3 - 6.0 mmol/L]
s-tsh - 2.53 uIU/ml [0.35 - 494 uIU/ml]
s-ft4 - 14.1 pmol/L [9.0 - 19.0 pmol/L]
Test total - 6.0 nmol/L
SHBG - 33.30 [11.1 - 78.1 pmol/L]
Free test - 112 pmol/L [180 - 739 pmol/L]
Prolactin -s 63.0 ug/L [4.3 - 30 ug/L]

My age - 22
Weight - 73kg
Body fat - guessing 16% ish
Bone mass went down during the time 2019 til now

The doctor said I'm hypogonadil but the one I'm rather worried about is my prolactin levels and stories of tumors etc, he wants to dive deeper in the origin of to why my prolactin levels is this high because he thinks i may have Prolactinoma but my medical aid is exhausted and can only do the tests/scans next year, in the meantime he wants to put me on trt to help my symptoms improve, he prescribed me androgel but then later noticed the clinics near me don't have it and the cost for it is a bit high compared to depo testosterone, depo test is better because its cheaper, more practical and absorbs better in the blood stream but my doc says otherwise. I'm going to contact my doctor soon that he prescribes me the injectables because they are the only type of exogenous testosterone near me but my question is should i talk to him about hcg ?[I still want kids someday] why didn't he mention this to me isn't it needed to keep fertility ? some research says no but stil,

Should I try TRT for my age or not ?

Looks like ill be on trt for life if I choose that route but the necessity to get my test levels to normal is a priority to me because it feels like my hormones are bipolar or something :lol: this is a big step, I just want to dive into this well educated

How should I use hcg while on trt for life ?, what dosage ? Should i only start hcg if i want kids? Should he try to sort out my prolactin levels first and after that is sorted will my test raise again?

I apologize for all the questions I know most of the questions i should rather ask my doctor but like i said I want opinions/suggestion and knowledge from other people as well

You to young to know for certain if you want kids or not, meet the right woman and you will see how your thinking changes. I would not go down the TRT route.. Primary or hypogonadotropic hypogonadism can be treated with SERMS and hCG. Where is your LH and FSH bloods? We need to see if its related to the pituitary or the testes not responding to signals and I'd be surprised having high prolactin without elevated e2, have you had your e2 tested? Your SHBG is also pretty high, what is your bodyfat like, really 16 percent?

Get your pituitary checked and take it from there. Pricey for a scan yes, but at 22 and in normal physical shape you should not have prolactin so high without any other conditions. Some antipsychotic meds can raise prolactin but I'm sure your doc would know what meds you are on.

There's no point just going on TRT if you have some underlying condition that needs to be treated. That's like putting a bandage on a bullet would and popping some painkillers.

You could try some cabaser/dostinex which will definitely bring down your prolactin and then do blood work after 4-6 weeks but all that will do is show how your testosterone can recover with lowered prolactin.

Hi tnx for the quick replies guys !!

I'll talk to my doc about everything you guys mentioned this week.


Oh sorry my e2 levels are 74 pmol/L [40 - 161] I always had low e2 +/- 50 pmol/L so its defintaly elevated for my standards but not by much.
My bodyfat percentage is an obsirvable guess [I have visible-ish abs but that doesnt justify if I'm 16% or not so I could be wrong].

Im not taking any opiods or antipsychotics and yes my doc knows that :)

I have no idea why he wants to put me on trt without taking my Lh and fsh first to see what's to blame for my low T high prolactin but I'm sceptical about going down the trt route without treating the actual issue and I mentioned that to him. During the meantime since I posted my issue I've done more research about everything and I concluded that TRT is the last resort. I will mention to him about cabaser/dostinex but I think i will postpone any treatment till the new med aid kicks in to have a solid understanding of what's going on but my low T symptoms are making me feel like crap but I'l soldier on !

No, he did not, I'll mention this to him because I'm actually keen to see myself when I'm getting my new bloodwork done, does prolactin supress dhea as well ? Anyways the bloodworks will confirm/deny this
