Any fat loss agent will actually raise blood sugar, due to the mechanism of action. They are used to liberate stored fat, mobilize it into the blood and then be oxidezed by the body as energy. Issue is they can improve insulin sensitivity due to being in a caloric defecit and if your wife is a hyper secreter of insulin then it may be an issue. But if she is using complex carbs combined with a fat source and protein source altogether it will slow down the rate of digestion and the rate at what carbs hit the blood stream and secrete insulin.

When does she feel her lows mostly? After eating carbs on their own?

Thank you for the explanation. As far as we are aware, she has slightly raised insulin levels, however her low glucose levels are mostly due to an underproduction of glucagon.

She mostly experiences lows after consuming foods containing carbs or sugar without any fat or protein sources.
When consuming too much carbs along with proteins she also experiences lows, however not nearly as bad as without any source of protein or fat.
She however does limit her intake of simple carbs and tries to consume vertually no processed sugar, including cane sugar, when consuming carbs she does try to limit it to complex carbs as advised in an attempt to keep her blood glucose levels more stable.
Which has been working very well.

Well there you go. She needs to be eating a balanced diet in a calorie deficit and then she wont have issues and needs to combine protein carbs fat and fiber and these issues would be gone majority of the time.
