I wouldn't recommend lifting twice a day as your body needs time to recover, this is when muscle growth occurs.

Nothing wrong with fasted cardio in the morning although some might say you should lift then due to apparent elevated test levels in the morning.

Personally, I lift in the evening and then do my cardio straight after.

I've always had great results with Max-OT. I don't follow it as strictly as I used to, but still apply the same principles. All Max-OT really is, is a way of training that allows enough rest between sets to go as heavy as possible. If you still have some juice left in the tank after your 6th rep, then you are not going as heavy as you should. At least, that's the way I see it.
